If you are travelling to a foreign country, you are likely to require vaccinations relating to the country you are visiting for your holiday.
You will need to complete a travel questionnaire. Please complete this and email to email@example.com or you can collect and return the form to either of the surgery sites - Church Walk and Church Street.
This form asks which countries (and the areas within countries) that you are visiting, to determine which vaccinations are required. Further information on vaccinations required can be found on the links below.
The travel questionnaire needs to be completed 8 weeks before your holiday start date. The Practice Nurse will review the form to check which vaccinations are needed and when they need to be done. The Receptionist will then arrange an appointment for you with the Practice Nurse. If you do not hear from the practice within a week after returning the completed questionnaire, please contact the surgery.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge as not all travel vaccinations are included in the services provided by the NHS.

PLEASE NOTE - The surgery are only able to offer basic travel vaccinations. If you require Rabies, Hepatitis B, Japanese Encephalitis and some other non-basic vaccines you will need to source these privately. Please contact the surgery for further details.
Demonstrating Your Covid 19 Vaccination Status When Travelling Abroad
From 17 May, you may be able to show your COVID-19 vaccination status as proof of your status when travelling abroad.
You can access your COVID-19 vaccination status through the free NHS App from 17 May. You can access the app through mobile devices such as a smartphone or by tablet. Proof of your COVID-19 vaccination status will be shown within the NHS App. We recommend that you register with the app before booking international travel.
If you do not have access to a smartphone and know that the country you are travelling to requires COVID-19 vaccination status, you can call the NHS helpline on 119 (from 17 May) and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine. We expect the letter to take up to 5 days to reach you.
Please do not contact your GP surgery about your COVID-19 vaccination status. GPs cannot provide letters showing your COVID-19 vaccination status.
