If you are travelling to a foreign country, you are likely to require vaccinations relating to the country you are visiting for your holiday. You will need to complete a travel questionnaire. Travel questionnaire forms are available from either of the surgery sites - Church Walk and Church Street.
This form includes which countries and areas within countries that you are visiting to determine what vaccinations are required. There is further information about countries and vaccinations required on the links below
You will need to complete the travel questionnaire 8 weeks before your holiday start date. Please hand the completed form to the surgery receptionist (at either surgery site) The Practice Nurse will review the form to see what vaccinations are needed and when you need them. The receptionist will then contact the patient to advise you of this and to book you in for an appointment with the Practice Nurse. If you have not heard from the surgery a few days after handing the completed questionnaire in, please contact the surgery so that we can look into this and see what vaccinations (if any) you require
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
PLEASE NOTE - The surgery are only able to offer basic travel vaccinations. If you require Rabies, Hepatitis B, Japanese Encephalitis and some other non basic vaccines you will need to source these privately. Please contact the surgery for further details.
